MIPS (Merit-based Incentive Payment System): How does it work?
MIPS measures are a set of quality measures used by the Centers for Medicare & Medicaid Services (CMS) to evaluate the performance of participating clinicians and groups under the Quality Payment Program (QPP).
We partnered with Zotec to select the MIPs measure that fit Coaxion's workload and were the most efficient for doctors to complete. We must complete 75% of each measure during the year to be compliant with the guidelines.
RFI (Request for Information): How do I submit?
The process to enter an RFI, quality inquiry, and addendum are the same. We utilize a ticketing system through a portal with Zendesk. It’s a very simple process:

How does the QA Process/Peer Review system work?
A QA case is generated when an outside physician notes a discrepancy on a case, or when a discrepancy is found during PR. When a QA ticket is created, the study is then sent back to the original reading radiologist, who either agrees or disagrees with the QA findings. Conflicts are submitted to a member of the Coaxion QA Committee for resolution.
For the purpose of achieving quality outcomes, if the second reading of an exam yields a discrepant finding which is considered clinically significant, (a RADPEER designation of “b”) an addendum is completed on the patient's report and a resolution document outlining the details is sent to the facility.
All studies and their final RADPEER scores are archived and tabulated. The data is used to evaluate radiologist performance levels, both individually and as a group. We look for trends in the data and use it to inform our radiologists of areas where they may benefit from additional CME. In rare circumstances, when a radiologist’s QA rate remains unacceptably high, they may have the scope of their work restricted or they may have their contract terminated.
Peer review is conducted through OnePacs. Each month, final interpretations are reviewed internally. The studies are randomly selected and assigned to a reading radiologist that did not initially read the study. The number of studies assigned to a radiologist correlates with the number of studies they have read for that month. Peer review results will be shared quarterly.
Where do I enter and/or call for critical results?
The following steps are required to ensure JCAHO requirements: After you have notified the ordering physician/ED, you will select “Flag study as requiring results reporting”. You must dictate the critical finding information within the Impression of the report, as shown below:
Template:
IMPRESSION: Critical finding of (finding) was communicated to (ordering physician name) by (reading radiologist) at (time & time zone). Dr. voiced understanding.
Example:
IMPRESSION: Critical finding of acute hemorrhage was communicated to Dr. Coaxion by Dr. Burton at 12:27 CST. Dr. voiced understanding.
Occasionally, a Radiologist is unable to reach facility personnel to inform them of a critical finding. When this occurs, the Radiologist may request that a support team member help reach someone to make the notification.
- If the Radiologist has already made an unsuccessful attempt to make the critical finding notification, the Radiologist can finalize the report, call the mainline (469-677-7430) and request assistance from a Coaxion Support team member.
- The Coaxion team member will immediately begin trying to contact the ordering provider. If a team member is not able to reach the ordering provider, they will contact the facility’s radiology director. Once the team member reaches someone, they will connect the radiologist and the critical finding recipient. The Coaxion team member will not relay any clinical information and will only facilitate the notification.
- Once the radiologist has made the notification to the facility, the radiologist will complete an addendum with the required details of the notification (listed above) and flag the study for results reporting.
